Insurance Coverage

Converge is now accepting several types of medical insurance, contact us today to see if yours is in our network. You will be responsible for any co-pays and deductibles per your plan and these can vary.

FEES for Direct Paying Patients (not covered by insurance):
For an initial evaluation (up to 60 minutes), our fee is $250.00. Your initial follow-up visit will last up to 30 minutes, and will cost $125.00. The fee for (up to) 20-minute med management visits is
$85.00. Other miscellaneous services such as filling forms, telephone correspondence, prior
authorizations, court hearings, etc. requiring more than ten minutes of time, will cost $40.00 per ten
minute interval. Fees may be subject to change. If our fees are to change, we will provide you a
thirty day notice to alert you to the change.


CANCELLATIONS AND NO-SHOWS
If you must cancel or reschedule an appointment, We require at least 24-hour notice (weekends not
included). If your appointment is on a Monday, the cancellation must be made by the same hour on
the preceding Friday. Cancellations that occur with less than 24-hour notice or failure to show to
an appointment will be charged a cancellation fee for the session.


PAYMENTS for Direct Pay Patients:
We expect payment at the beginning of each session, unless we have agreed on other arrangements.
We accept cash or personal check, Venmo, PayPal and major credit cards. Checks should be made
payable to “Converge Mental Wellness Associates, PLLC.” If payment is 60 days past due, We
reserve the right to utilize legal resources such as collection agencies or small claims court in order
to obtain payment for our services. Checks returned for insufficient funds will incur a $25.00 fee
for processing in addition to requiring replacement payment. Termination of services may result
from failure to provide payment as agreed upon.
